Under Massachusetts state law and ACA standard HW.5, all campers must have on file before attending camp:

• Health history form, completed by May 1st

• Physical completed within 24 months preceding your camper's attendance at Summer Fenn

• Current Immunization Record

All medical information must be completed thru your CampInTouch account. Your account will be accessible once we have enrolled your camper.

The Health History must be completed online by the camper's parent or guardian. This form does not need to be downloaded and filled out.  Online is the best way to complete this part of the medical paperwork.

The Physical Form is available to download to bring to the Doctor or a standard form from the physician's office is appropriate.  A current, up-to-date Immunization Record must accompany the Physical Form and either a printed copy from your Doctor or the camp downloaded form is acceptable.

Authorization forms are also required for the administration of prescription and over-the-counter medications. This form must be completed online for any prescriptions or over-the-counter medications to be taken while at camp.  ALL prescription medication must be in their original container with complete camper’s name, address, instructions and dosage.

While the medical part of the online process is extensive, it will only have to be updated after year one, which will make paperwork that much easier in the following years.
